Register with this Practice

To register with the surgery

  • fill out an online registration form or download and complete the forms
  • visit reception during surgery hours and collect a paper registration form
  • we require evidence of identity to provide certain services (refer to registration policy regarding ID). Until we have seen your identification you will not be registered at the practice.

New Patient Policy

Where it is clinically appropriate and practical to register, we now accept new registrations from patients who work in the local area but reside outside of our registration area. Patients registered this way would not be entitled to home visit from the practice, however they will be able to contact NHS 111 in order to be seen by a practice closer to where they live.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

You will be assigned to a doctor who will be your Named Accountable GP. 

You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the surgery but it will not always be possible to see the doctor of your choice on a particular day or time.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Your medical records remain with your registered GP but we will provide you with full medical care while you are in the area. If you leave the UK for a period of more than three months the doctor is no longer responsible for your medical care and may not provide repeat medication in your absence.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Register at our GP surgery with a paper form

You can register with us if you are homeless or of no fixed abode but reside within the practice boundary. Further information can be found on the NHS website.

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from our Practice for up to 14 days. After 14 days, you will need to register as a temporary or permanent patient. Contact reception for further information.

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the local Practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
